The "crossed-out eye" icon seems to represent missing files or perhaps it just means it can't create a thumbnail view of the video.

- I assume as well as the file extension type you have also allowed the MIME type e.g. video/mp4, audio/mp4 ?
- Try a direct hyperlink <a href="myvideo.mp4">my video</a> type link instead of embedding.
- Use a media play extension (even for testing), for example: https://extensions.joomla.org/extension/allvideos/
- Use standard HTML <video> tag (refer https://www.w3schools.com/html/html5_video.asp)
- Make sure JCE is updated - a new release came out yesterday 24/09/2024 (Australian WST)
